By double-clicking, I mean to save the file to disk and then dbl-clk in the Windows Explorer.

Yes, that works fine. I don't see any of these problems when I save to disk and open it. Unfortunately, the client wants the "Open" option as well.	[reply]

Then it looks as though your browser is changing or passing something the app doesn't like. I would try replicating in Firefox using an header sniffing extension and see if 1) it works in Firefox and 2) Sniff your headers to see that they are coming across correctly.
