I added "warn $self->filename" to "sub filename_without_inc" in Devel::NYTProf::FileInfo:
sub filename_without_inc {
    my $self = shift;
    my $f    = [$self->filename];
warn $self->filename;
These warnings lead me to believe the problem is not in my code:
