Something is killing your script before it loads Carp. Is perl really at /usr/bin/perl? Does the script have the right permissions? Are the line-endings appropriate? Is the web server configured to run perl scripts?

You may get some insight by examining the server logs. Also try the CGI troubleshooting guide, the CGI MetaFAQ or the poorly named Idiot's Guide to Solving Perl CGI Problems
