Added the final four. Not sure i'd consider the first two "sins," though. The former is demanded, at times. The latter is an honest mistake. There's a difference between those and sins, right? :)

For me, it’s mostly inadequate testing.

Isn't that what users are for? Er, i mean, "beta."
