Thanks for sharing this. I too completed the survey but was torn about how to respond, especially to the questions regarding what kind of support would I be looking for (as a hobbyist programmer new to Perl). Personally, I prefer the "asynchronous" nature of things like mailing lists that allow for more thoughtful exchanges at times that are convenient for me. But we obviously have that resource right here so I'd be hesitant to recommend something that competes with this site. Because of Perl's age, there are dozens of good books available ranging from introductory materials to in-depth examinations of Perl's more advanced features. What I ultimately recommended was something like online courses on edX or Coursera that offer ability to interact with a live instructor and students and possibly have assignments or projects for new Perl programmers to apply their skills. I know pulling something like that isn't easy but with so many options already out there, it was the only "novel" one I could think of.

Not sure why I feel like I just did my annual Personal Development Procedure. Nonetheless, came up with at least one genius level idea so I am sure there is utility amongst the futility.

Question 7 was a bit tricky to answer, as it kind of depends a lot on how you've used existing formats, but the question asks you to rate how often you would use something, which is not yet developed. However, I proceeded to give a best guess, assuming all working as imagined.
