Update
removed link, don't wanna reward the spammer...
update
something seems to be broken here: from What is moderation?
For Anonymous Monks , and those registered users who have elected not to see unapproved posts, each section's main page will list only approved posts. It is possible for registered monks to change their user settings so as to see both approved and unapproved posts in each section. (Even then, they will be listed separately.) Registered monks should read Miscellaneous Settings for details on how to make unapproved content visible.

Ahhhhhhh ... OK now I got it! :)
For Anonymous Monks "main pages" like SOPW show only approved posts, BUT Recently Active Threads and Newest Nodes show always everything.
So the approval mechanism doesn't help blocking spam, since bots parse all index pages.

When not logged in, I have not had problems viewing unapproved nodes. I could be wrong, but I believe that the only nodes not viewable when not logged in are the reaped nodes.
